Disclosure Statement of Terms and Conditions on Use of Automatic Teller
Machine Cards
This disclosure is provided to
members/consumers according to the provisions of the Federal Electronic Funds Transfer
Act. As used in this Disclosure, the words "we", "us" or
the "Credit Union" refer to the employees of Metro North Federal Credit Union;
the words "you" and "your" refer to the member ( and joint account
holder, if any); the words "ATM Card": refer to the Credit Union access card.
This Disclosure applies to Automated Teller Machine (ATM) services provided by Metro North
Federal Credit Union. Please read this Disclosure in it's entirety because these
disclosures may affect you.
1. YOUR LIABILTY FOR
UNAUTHORIZED TRANSFERS.
Tell us AT ONCE if you believe your ATM
Card and /or PERSONAL IDENTIFICATION NUMBER (PIN) has been lost, stolen
or used without your permission. Telephoning us is your best way of keeping your losses
down. You could lose all the money in your account(s). If you tell us within two business
days, you can lose no more than $50 if someone used your ATM CARD and/or PIN without your
permission.
If you do not tell us within two business days, after you learn of the loss or theft of
your ATM CARD and/or PIN, and we can prove we could have stopped someone from using your
ATM CARD and/or PIN without your permission if you had told us, you could lose as much as
$500.
Also, if your statement shows transactions that you did
not make or authorize, tell us AT ONCE. If you do NOT tell us within 60 days after the
statement was mail to you, you may not get back any of the money you lost after 60 days.
If your periodic statement shows transfers that you did not make, notify the credit union
at once. If you do not notify the credit union within 60 days after the statement was
mailed to you, you may lose any and all funds after the 60 days if the credit union can
prove that the loss could have been prevented had you notified the credit union in time.

If you believe you ATM CARD and/or PERSONAL IDENTIFICATION
NUMBER has been lost or stolen, call: 1-904-757-5562 or write to:

3. ATM CREDIT UNION BUSINESS
DAYS. Our business days are Monday through Friday, 7 Am to 4:30 PM (Eastern
time). Holidays are not included.
4. OUR ELECTRONICFUND
TRANSFER SERVICES. Types of available transactions and limits on transactions:
Your Share/Share Draft Account may be accessed by you ATM card for the following:
(a) Deposits
(c)
Transfers
(e) Inquires
(b) Withdrawals
(d) Point-of-Sale
You may use your ATM CARD to: Withdraw funds from your Share and Share
Draft Account.
5. LIMITATIONS ON ELECTRONIC
FUND TRANSFERS.
(a) ATM
Card. you may withdraw up to the daily limit each day.
The daily limit will be
in effect 7 days a week.
(b) ATM
Deposits may be subject to a 48 hour hold. Reg E requires $100 next day release of those
funds deposited.
6. FEES AND CHARGES. Charges
for your Metro North Federal
ATM Card privilege are listed below:
(a) You will be given 10 (ten) free transactions per month.
(b) We will charge $1.50 per transaction fee.
(c) A replacement card is $5.00.
(d) A replacement PIN is $5.00.
(e) A $20.00 service charge will be accessed for transactions not covered by sufficient
funds.
You hereby agree to authorize the Credit Union to debit any of your
accounts to cover any or all of the above charges.
7. OTHER TERMS AND
CONDITIONS. Your account(s) may also be governed by other terms and conditions
preciously set by us. if any of those terms and conditions conflict with the terms and
conditions of this disclosure statement, this disclosure statement will prevail.
